Change Monitor
Small changes can be signs of illness
For frail older people, the early signs of illness can be difficult to detect before the condition progresses and may lead to hospital admission. The Change Monitor continuously tracks the citizen’s vital signs in their own home and can alert healthcare professionals to subtle changes.
The aim is to detect deterioration earlier, allowing action to be taken before the condition becomes serious.
Clinical need
Demographic changes are leading to a growing need for treatment and care for older people, placing significant pressure on the healthcare system.
Frail older people often have complex health conditions, characterised by multiple chronic conditions and the use of several medications, which can mask and/or exacerbate their actual condition. As a result, detecting the early signs or recurrence of illness in a timely manner can be difficult within current healthcare settings and requires a high level of professional expertise and experience.
Solution
The Change Monitor is a wearable medical technology designed to support early intervention in the citizen’s own home and help prevent hospital admission.
By frequently monitoring vital signs, the technology can alert healthcare professionals in both primary and secondary care to subtle signs of emerging or worsening illness. This can support care staff in their decision-making while also providing physicians with a better basis for clinical assessment and earlier intervention. In the longer term, the technology is also expected to help prevent hospital admissions and readmissions.
